Probationary Assessor Supervision Form

Supporting & Developing Volunteer Education Teams, Member Education

    1. Follow the access link and instructions provided in the Assessment Memo. Any preassessment information will be prepopulated by SLSSA Member Ed staff.

    2. Use a device such as a phone, iPod or laptop to complete the form online during the assessment

    3. Complete all relevant fields on each 'page', and click 'Save'

    4. Be mindful to providing constructive feedback and comments, as a copy will be automatically provided to both the Probationary Assessor and Lead Assessor of the Assessment

    5. Discuss the assessment and feedback provided to the Probationary Assessor before submitting the form

    6. Depending on what Probationary Assessor Assessment number and the recommended outcome, will determine the next steps

  • Access to the form will be provided via the Assessment Memo as the same online form is to be used for both Assessment 1 and Assessment 2 for each person.

Purpose

This form is to be used to capture feedback from a Lead Assessor, for someone undertaking the Probationary Assessor pathway and support them on their journey to becoming an Endorsed Assessor.
